I have talked to or emailed hundreds of pituitary patients since I had my endoscopic surgery with Dr. Shahinin in Los Angles in 2000. He devised his own instruments for endoscopic surgery and is considered one of the best surgeons (I think THE best in the world) for pituitary tumor removal. It was his opinion that if the tumor could be removed surgically that it was the preferred way to go. (In one coversation with him, he was seeing a person who had 8 previous surgeries!) I went to him because I believed that his method had the best chance of seeing all the tumor. In my case I saw the video of my surgery. He showed me that the tumor had grown into areas not seen on the MRI and he said he would not have seen the tumor which had grown outside the sella tursica and was laying up along my brain had he still been operating the transphenoidal method. (He operated with that tecnique for about 600 patients before he invented his own instruments for endoscopic surgery). The tumor grows back because it was not all removed. These things are aggressive and unless all of it is removed it will regrow. Another endoscopic surgeon I have heard good things about is Dr. Jho (who actually invented the endoscopic method of removing tumors.) I have not heard raves about any other surgeon using a fully endoscopic method even though other surgeons are taking up this method. You can call me if you want: 541-757-9663.
