I have noticed that after the Access Violation error the icon does not
turn blue unless I open the app, go to the config panel and then hit
Apply , then the app functions fine again.
I have also noticed that in the application log the line - reading
setting from registry is where the app is hanging, I then ht apply and
the app continue with writing new setting to registry and then locating
the server etc etc.

Hope this helps you hunt down the bug !
