OK - I have unzipped the CustomBrowse download file into the plugins folder and restarted - and woopee I now have Custom Browse. But all of my old menus are still visible so perhaps the problem all along was the previous installation which still existed somewhere - goodness knows where - and was stopping the install via the extension downloader.
I completely removed Squeezebox Server, and manually deleted all of the directories I could find but it was still lurking somewhere.
