Well, the "keepalive" functionality is now completely rewritten and it's waiting for review at the App Store for a while. That said: There is at least one part of the issue where - after some discussion with Apple - there is probably a bug or changed behavior (may be intended) about running behind the screen saver forever (background tasks never end as they otherwise do). I filed a bug to clarify things with Apple and I tried to work around it. Hope it helps. The big issue has alway been that this is so hard to reproduce for me. I've seen it a few times, too, but in 99% of my use it really goes to sleep even with the current release. Well, it's a server issue. The first behavior is as it's being sorted by the server, the second one is when iPeng sorts for itself. Unfortunately, always sorting in iPeng is not an option due to 1. Performance (you might have noticed that the "All Songs" list can act a bit strange while being in the process loading larger lists of tracks 2. iPeng doesn't have all the necessary information. You may have e.g. "ARTISTSORT" or "ALBUMSORT" tags in your library that are being honored by the server but not transmitted to iPeng.
