I second the recommendation about the Clear&Rescan. "Flush Cache" in iPeng only makes iPeng re-read everything from the server. But that will not clean up duplicates. Duplicates are always in the library itself, iPeng doesn't aggregate different states from the past, it always only uses a single snapshot of the library so if you are seeing dupes, they have been there the last time iPeng read this data.
