Well, as you can see it happens during the scan :-). Once the main scanner is done indexing music files and their cover art, it pulls a list of albums without cover and tries to grab them online.
Where they go depends on your configuration: you can define a path where they should go (and where you could edit them etc.) like for the artist pictures. It's actually a setting of core LMS, which as been there for a while already. If you leave that field blank, files will be stored in the cache, where you can't access and edit them, and where they will expire at some point...
Other observations: The only artists things work I only get the main artists .

Yes, the plugin does lookups on the internet, downloads the images, pre-caches them etc.
What does the profile export relate to it has more items than I have tracks ?
Profile export? That would be SmartMix. More items than you have tracks? That would be wrong :-(. But then there's no really new SmartMix release, that should not have changed.
