However if I put the command c:\utils\xnview.exe in quotes
the icon reappears again. There is no need for the item in the
command field to contain a space.

That makes me think the icon displaying routine is looking at the
entire command line:
c:\utils\xnview.exe "%windir%\Gone Fishing.bmp" ; no icon
c:\utils\xnview.exe "c:\windows\Gone Fishing.bmp" ; icon shown
"c:\utils\xnview.exe" "%windir%\Gone Fishing.bmp" ; icon shown

So if there is an %envar%, but no quotes around the command,
then the icon display routine finds it difficult to know how much
of the line to select when looking for its source exe.
The quotes around the command help it select the command (exe)
part of the line.

On the other hand the executing routine has no such problems
because clicking the button works as expected, no matter which of
the three permutations shown above is used.
