I reinstalled with the configuration files set to the user folders.
Still no TaskBarBar. If I change the name of TaskBarBar to something
else, I can see the bar (as a floating bar) and it works correctly.
When I change the name back to TaskBarBar, the bar disappears, so PPro
clearly recognizes that it's a special case. But it doesn't show up in
the list of toolbars on the taskbar.

I tried this on my xp box and it worked there, so I think I'm doing
the right steps.

All of the above work. Some of them didn't seem to work at first, and
I would try something else and then come back to it and it would work.
Probably just a mistake in my setup, but perhaps there is some
flakiness there. Once I got it working it seemed reliable, though, so
probably just operator error. 

For the last one, just to be clear, I attached "*keys abc" to a
button. This works fine, when I click the button the keys get sent to
Notepad (or whatever window is active).

I'm not sure what you mean by "send sa 'abc'". Was that just a typo,
or is there some "sa" option I don't know about?

One thing you might want to mention somewhere, is that vista users
need to download and install WinHelp from microsoft. WinHelp is not
installed by default on vista, and since your help file is winhelp
format, you need to get winhelp installed to be able to see help. Of
course putting this in the help file won't be much help -- maybe the
readme?
