Somehow, the desktop is lost and has to be re-built at every startup.
So my question is now: Does this mean my harddisk is on the brink of collapsing, or is it something that can be fixed?

It may mean the desktop is corrupt and the Finder is taking a lot of time thrashing through it. Try doing a desktop rebuild.
