Palo Verde provides the following response:

(1) Does your facility use "Trip Tickets" to access the RRA/RCA? Yes

If so,

A) How many levels or types of tickets based on type of work or activity to be
performed are used? Three: White ticket for RA entry, Yellow for HRA, and Red
for LHRA

B) What type of radiological briefing is given for each level, (for example
self-briefed or RP briefed)? The white ticket does not require a “formal”
brief; however, many radworkers check with RP staff to ensure dose rates,
contamination levels, etc. The Yellow and Red require a formal brief.

(2) How are work control and the "dose budget" integrated into the RRA/RCA
access process? Work groups are required to come up with dose estimates for
jobs and accumulated dose per job is tracked and discussed at the site’s daily
Operational Focus Meeting. If actual dose is > 25% above or 50% below the
estimate a condition report is generated and a review performed.

A) How is the dose budget maintained and updated? Daily thru an electronic
report from the access control system
