Prior to using the SAMs, we used tool monitors that relied on door operation to automatically start the counting cycle. The described situation is an inherent feature of the SAMs and from a corrective action perspective you would be faced with prohibiting workers from using unless they had direct RP supervision or perhaps implementing an advanced radworker qualification to use a tool monitor. We have had occasions where workers place material in the monitors, and do not push the start button, or place items in the SAM during a background count. While an attentive worker can observe fault messages, it appears the messages from the SAM-11 are less distinct than those produced by the SAM-9. Some workers have passed items through SAM-11s without an actual count. Does anyone else have similar experiences? How have you responded?
