Jim, the two large plans that I am familiar with do not disclose any diagnostic type information on the EOB's - they limit the 'EOB level' exposure to the name (and possibly the location) of the provider, date(s) of service and the financials. Right now we are considering that a 'reasonable' practice under minimum necessary - thx- b
