Go to View > New Guide and type in the co-ordinates. You can also place a
guide precisely by holding down the shift key and watching the Info palette
readouts.
A lot of us (OK, just moi!) are pushing for a Guide Manager that can make
and position vertical, horizontal and diagonal guides as well as give us the
option of making a selection from the guides. Put your vote in on the Adobe
site on the features request page.
