Well, I have built book.html in my working copy as follows:
~/producingoss-trunk/pt-pt$ cat book.xml | xsltproc
../tools/html-stylesheet.xsl - > book.html

In the XML file, any of the options works:

<book id="poss" lang="pt-pt">
<book id="poss" lang="pt_PT">

This is the output:
