I mainly work for SMBs. The way I tend to operate is for the client, the users and I to discuss their requirements and between us we come up with our best guess as to what they need. Often that is converting an existing manual system to a computer based one. I tell them that we will probably need a mark 2 version when they have found what they *really* want as opposed to what they thought they wanted but if we get it nearly right then that won't be too big a step. I then build a non-working prototype and write a spec with screenshots. That spec is the definitive version that the fixed price I quote is based on. I then usually get a trickle of requests for enhancements as they realise where additional features would be useful or their business model changes.
