Stuart,
        are your missing holes single layer (top or bottom) pads by chance?
A couple of us have seen this issue a few weeks ago. Unfortunately we
haven't formulated any definitive understanding of the issue yet, it appears
to rather random and doesn't effect every design with the same
characteristic pad. The temporary work-around we used to get the output
Gerber right was to change pads to multi-layer temporarily while generating
Gerber drill drawings.

 We have a board where the gerber drill drawing is missing
 holes, the hole size for a bunch of vias are listed a 0. The
 NC drill data is fine and the print/preview drawings are
 fine.
