DigiKey ( www.digikey.com ) has exactly what you are looking for on page 588 of their most recent catalog.
The resistor is made by Panasonic and can be purchased from DigiKey in quantities of 10, 100, or 200 ( DigiKey part number P4.0TCT-ND for 5% and P4.0MCT-ND for 1% ).
