This is one where hopefully PCAD can influence DXP. If I recall correctly (it has been a while now) PCAD placed designators, or any other attribute, according to where the attribute string was placed in the library symbol or land pattern. After placement they could be moved but you could set the most likely/desired position when creating the library symbols and land patterns.
