Actually different sizes are very easy to do in many ways.

Through drag,n drop:
Change the settings under [ Tools->Setup->Drag,n Dop -> Button Size ]

Through context menu:
Right click in the menu [Add Image] or [Add Movie] and then creating the
size you want

Through Mouse:
Click on the preview on the lower right corner ( make sure you get the
exact yellow point ) and move the mouse

Different shapes are available for images, and movies.
1) Unbutton you button if needed.
2) Add Mask
3) Play around with the settings, like shape, frames, colors,
transparency, rotation, position etc.
4) click [Okay]
5) Define as Button

I think what  you'll find is that you have a ton of creative
possibilities with QDVDAuthor. You can even animate most of the
settings, like color, positions, transparency, size etc. for image/movie
objects, or mask objects alike.
