Ok, So I finally went ahead and installed SpamAssassin. I'm not sure QmailAdmin is adding the flags correctally. I looked in 1 account I enabled Spam Detection in and the .qmail file has.

Is this correct syntax for a .qmail file.
No. Maildrop does not call spamassassin like this.
Do you have Maildrop installed? The line should look something like this:
|preline /usr/local/bin/maildrop /path/to/mailfilter
I did install the config for SpamAssassin in /etc/mail/spamassassin. Am I missing configs on QmailAdmin part or SpamAssassin part.
Looks like your probably missing the maildrop application and the maildrop filter. Here is my filter from an earlier post:
