i just recently setup a new box and i had the same issue turning spamd on.

make sure
QMAILQUEUE="/var/qmail/bin/simscan" appears on your /etc/tcprules.d/tcp.smtp
file
and that scanning is turned on in /var/qmail/control/simscan file
