just recently setup a new box using similar method

check out /etc/tcprules.d/tcp.smtp file

make sure
QMAILQUEUE="/var/qmail/bin/simscan"
appears on the 2nd line somehwere

and make sure spam scanning is turned on in /var/qmail/control/simscan file


#qmail-spam status
should also reveal whether spamd is running too

IIRC that is what i had to do.
