That's indeed peculiar. Like I said, I've no idea how your system came to have qmail-send and smtp2 directories under /var/qmail/supervise. I would move them somewhere else safe (as opposed to simply removing them) and see what happens:
# service qmail stop
# mv /var/qmail/supervise/qmail-send /root/.
# mv /var/qmail/supervise/smtp2 /root/.
# service qmail start

is any other way to fix or find this issue.

See if moving those folders remedies the problem. If not, I would suggest finding corresponding entries in the smtp logs to verify whether or not the duplicates are coming into the server twice or not. Use qmlog to search the logs (-lc option).

Are these duplicates intermittent, or always?
All or some accounts?

What is the load on the server?

Have you mentioned which versions you're running?
# rpm -qa | grep toaster
