One time when I was coming out of rehab the therapist forgot to put my  chest 
strap back on and as I was driving down the driveway, into a major street,  I 
felt forward and knew that there was nothing I could do.  My caregiver  was 
across the street opening up the van and could not hear me yelling.   Luckily, 
there was no traffic and I made it home alive that day.
 
It seems like no matter how many times I've run over my service dog she is  
always under wheels.
 
My family and other animals have learned to jump out of the way when they  
are to the click of the joystick.

I've  done the power chair two step when people's coats hang on my joystick. 
I've  backed over numerous animals. I backed down a flight of stairs by 
accident.  Amazingly, the chair and I suffered only minimal damage.  It just 
flipped 
 on it's back and slid down. Lost my backpack and some battery acid, but when 
 flipped back on my wheels I was only suffering from a hell of a rush.
Do  crushed feet count? I've got 5 so far.

Anyone ever accidentally drive  their chair into a pool or ditch, or fall out 
of your chair? 
I¢ve tipped sidewise or fallen  forward and couldn¢t sit back up, but have 
always stayed in the  chair. 
But I have bumped my control  joystick and scared myself being close to a 
pools edge.
