The self-adjuster is a possibility, but having had a couple of these
go, I think they usually fail the other way i.e. the pawl slips and
drops the clutch when you're not expecting it.
I now have a pedal with fixed adjustment, similar to the one described
on Jim's website.

My guess is that it's more likely is a fraying clutch cable, one or
more strands might have broken and be gradually unwinding inside the
outer sheath.
So the cable is getting stuck and not returning fully.
