We run Misys's PM product on AIX. For all of its canned reports a page eject is always performed before the report starts printing. For its letter-writing module, where we construct letters and forms with no wasting of paper by suppressing the page eject, the first page is always off one line compared to the subsequent pages.
For claim forms and patient statements, the PM product allows printing of test form prints until the printer is aligned to the preprinted form. Once physically aligned by the printer operator, the forms remained aligned, whether the first or subsequent. Perhaps there is a way of sending something like a page eject that does not eject a page in order to force alignment. The same printer (OKIDATA 320 Turbo) is used for both canned reports, user prepared letters and forms, preprinted claim forms (CMS-1500) and patient statements. Somehow the misalignment problem with the first page was solved for preprinted forms. However not for plain white tractor-fed computer paper where alignment is only necessary for cosmetics. Which ejecting a blank page takes care of.
