Dave, The short answer is no.
I have created an R:BASE form where I scanned a paper document, and used the jpg file from the scan as wallpaper on the R:BASE form, with input controls located at the appropriate places, so that the screen looked as much like the paper version as possible, but that's the closest thing I can imagine.
