Following up on Bill's answer, Dave, which actual "layer" of the PDF are you 
trying to import :
> the document/layout?
> the form/fields?
> the data that is input into the form?
 
I ask, not because I'm an expert here, but possibly to focus your effort a bit. 
 I am sure that it can be done, but I can't say what's required in time, money, 
tools, etc.  Again, not being an expert here but having done a little 
investigation in the past, I can say that you will probably need some one or 
more of the Adobe authoring/publishing products in order to make this happen.

Dave,

The short answer is no.

I have created an R:BASE form where I scanned a paper document, and used the 
jpg file from the scan as wallpaper on the R:BASE form, with input controls 
located at the appropriate places, so that the screen looked as much like the 
paper version as possible, but that's the closest thing I can imagine.
