Tim -- More and more, I think you should get Rivendell to build it. They're certainly not cheap, but they always do a good job -- as someone indicated, their frame and bike packing is an art form in itself (and I'm completely serious here). I'll be very interested to see the resulting build.
Just call Riv and tell them what you need and let them point you to the right person. I'm very finicky, but I'd trust them to choose the right person to build a bike for me. PS: please take photos of the packing once you receive the frame.
