i've got thumbies on my moosies, and i've moved them in a bit from 
right-up-against the grips, and even that way i can often shift simply by 
leaning my hand in at an angle.  if i'd left em where Rivendell put em, i could 
have shifted w/o moving my hand in the slightest, though my big hands mean i 
need all the grip space, so i did move them in a cm or two.  

but i find myself shifting with the side (push) or last joint (pull) of my 
index finger mostly - both pushing and pulling are pretty easy.  the largest 
hand contact patch (and strongest grip) runs from thumb-to-pinky and not 
thumb-to-index ... i tend to hold this way naturally from lots of martial arts. 
 so maybe it's me, but the small shifts i make with my first finger don't seem 
to destabilize a darn thing.  probably as as the two-lever push-click shifters 
(don't know what they are called) i had on a Gary Fisher 29er.
