Well not really a great solution, but what I am going to do is
surestream at near the average bandwidth I get from the site.  For
instance, with my 56k, I normally average a 26k stream rate.  Therefore
I will encode surestream at 24k, setting the audio for 5k, with the
remaining 19k for video.  At this rate, its about the best that I can
do now.  I am using real producer 7 plus, and it allows me to have
greater control of the encode settings. Note that I change the default
20k video setting for 28k users to 24k.  When its surestreamed, I get
the following stream rates:

SureStream Audio Codecs:
           5 Kbps Voice (RealAudio 5.0) 8000 Khz
           5 Kbps Voice (RealAudio 5.0) 8000 Khz - Backwards
          Compatibility Stream

Stream: 1 video/x-pn-multirate-realvideo logical stream

      MIME type: video/x-pn-realvideo
      Max Stream Bit Rate: 19.0 Kbps
      Dimensions: 192x144
      Encoded Frame Rate: 15.0 fps
      SureStream Video Codecs:
           19.0 Kbps (RealVideo G2)
           13.0 Kbps (RealVideo G2)
           9.4 Kbps (RealVideo G2)
           19.0 Kbps (RealVideo G2) - for HTTP streaming

Stream Pairings

      0.0 - 18.0 Kbps
           Stream0Bandwidth = 5.0 Kbps
           Stream1Bandwidth = 9.4 Kbps
      18.0 - 24.0 Kbps
           Stream0Bandwidth = 5.0 Kbps
           Stream1Bandwidth = 13.0 Kbps
      24.0 and above
           Stream0Bandwidth = 5.0 Kbps
           Stream1Bandwidth = 19.0 Kbps

With this setting, 28k users will normally get the 18kbps file,
which isnt too much different than the default 20k rate and should
handle lag better for 28k users.  Users with faster connections
will get the 24k rate.  Hope this helps the ppl who emailed me.
