That QLogic HBA should be what you're looking for.
Going slightly off-topic, if you're using HP c-class blades, you might
also consider, instead of iSCSI storage, their new MDS600 storage array:
http://h18006.www1.hp.com/products/storageworks/600mds/index.html
You use an external SAS controller mezzanine card in your blades, and it
basically allows you to attach large-form-factor disks in the MDS600 to
the blade. In effect, making a BL495 look and feel more like a DL385 G5p
when it comes to storage capacity.
I vouls have opted for an MSA2000sa, though, which is closer to my needs, but
I think they're basically the same thing, the MDS being larger.
Actually those have completely different ideology.
MDS600 will present individual disk(s) to controller(s) on the blade(s)
over SAS, parity etc will be handled by the raid-controller on the blade.
MSA2000 will do parity internally and present lun's to host(s) over
various protocols
