Qt4 is a completely different animal from Qt3. So much so, that porting RD to it requires almost as much work as writing the system originally. At one point, it was high on my 'todo' list. I had made some fair progress on it, but finally gave up on it as a waste of programming resources -- why invest man-months of time simply to 'go sideways'? One of the great things about Open Source is that Qt3 will always be available, so there is really no compelling need to make this change.
Will there ever come a time when Qt3 is no longer compatible with the latest Linux kernels or some other component of the OS?
I suppose if the Appliance becomes the principal way Rivendell is distributed, then it won't matter whether Qt3 is supported by the latest kernels, as the Appliance can include whatever kernel is necessary. There are IBOC encoders being sold that still use old 2.4 kernels.
