This could quite simply be avoided in almost all cases (except where saving the pfile causes the crash...) by implementing a signal handling system that autosaves pfiles in the event of a crash. I know that my players were quite unhappy when a crash reverted them back a few minutes. But, hey, if a crash saves everyone, no dupe eq either... (Just my 2c to throw on the dollar...)
