Specifically, who has what responsibility to do what? A simple erratum does not clear a document of a potential copyright violation, although it does at least correct the lack of acknowledgment.

A person has an obligation. The person does not fulfill that obligation. Nothing happens. The obligation (it's a policy in this case) is akin to a "paper tiger" [1]. Someone would have to either determine who was responsible for what for the two "what if" or sweep the matter under the rug [2].
