I see two aspects here: closing by choosing "Exit" or whatever or closing by WM. The former is application's responsibility and it should ask user if necessary. The later is WM's responsibility and it should ask user if necessary. Moreover, in case of urxvt, the former is not urxvt's responsibility since urxvt finishes when application that has been run finished. As such, it's responsibility of the application run inside urxvt to ask for confirmation.
