Yes I think Angela has it.  I think her second father-in-law has the same 
name as her paternal grandfather: Antonio Soares Coelho (her paternal 
grandmother is Maria Jacinta while her mother-in-law is Maria Jose Soares)

I really can’t make out note 3 except that I think it gives her name as Ana

Note 4: She died today, note dated 29 Oct 1983

Although it's abbreviated, I believe you are correct. Manuel Gonçalves das
Calhetas and Isabel Vieira.

Go to the Azores GenWeb here (make sure you bookmark this one; Rootsweb had
issues and one site is very old): http://www.worldgenweb.org/azrwgw/
Then look for the Maps button, pick Sao Miguel, and you will find a
Calhetas in Ribeira Grande. So the dad was from a different freguesia.
