In English:

 Exposed number 941 in the wheel of the town hall of this municipality on the 
first of the same month of September and year of 1862 at 10 hours in the 
morning in this same day was given to care for by the same town hall having the 
constant signs of the registration book of the same to Maria de Jesus...

Can't really make out the first few letters on the margin but it definitely 
says :
Filho natural de Maria Pacheco.

Mel, The main body *might* say that he was given to Maria de Jesus, wife of
Joao Pacheco Grande, to nurse. There's some words before that I'm
unfamiliar with, so a native speaker will have to help out here.

The side margin says the mom came and claimed him. He was the natural son
of Maria Pacheco. Of book 7, number 2. That's how I read it. I'm not sure
what it means. I don't see any legitimization books for the 1860s.  Cheri

I was hoping those who known Portuguese better than I do (which is pretty 
much everyone...LOL) can help me understand this record.  It involves an 
exposto named Luciano.  Is it saying that he was adopted by Maria de Jesus, 
wife of Joao Pacheco Grande?

Also in the side margin it seems to be saying he is the child of Maria 
Pacheco.  Is that correct? Is it inferring that Maria de Jesus is the birth 
mother, too, or just someone name Maria Pacheco? What do the numbers after 
stand for?  

I believe Joao Pacheco Grande is the son of Manoel Pacheco and Marianna 
Rosa, and grandson of my ancestors Joao Pacheo and Francisca Theresa.  
Maria de Jesus would have been his 3rd wife, Maria de Jesus Pacheco, 
daughter of Manoel Pacheco and Anna de Jesus.  (I have not connected them, 
but they could be related, too)
