Only one in my case.

Until OpenBSD supports 11n with MIMO, multiple antennas will only be used
for diversity which selects one antenna for transmit at a time (generally
based on which one has received better signal from the peer, I believe,
but the exact procedure is driver-dependent).

Always connect both antennas if you want to rule out problems with
antenna selection affecting wifi performance.

I can't advise on the speed or reliability but I have had no complaints

Works fine with hostap in an old eee 1000HA, although also not exactly high
volume traffic...

Which is the most used/recommended card?

This is not an easy question. Not because OpenBSD but because drivers
for wifi cards frequently change due to manufacturer decisions.

Are you looking for an integrated WLAN module, which is placed inside a 
laptop?

Or, are you looking for a mini form factor wlan card which is pci express?
