Wouldn't a truly physiological third stage involve waiting till the placenta
is birthed before clamping/cutting the cord?

Sue,
You really need to decide whether your patient wishes for an active or a 
physiological third stage. It can be dangerous to try and do a half-half 
sort of third stage.

If you plan a physiological third stage then you need to:

1. Dont clamp or cut the cord until it stops pulsating.
2. Put the baby to the breast as soon as possible, this will stimulate 
oxytocin release which works better than 10units Syntocinon ever does!
3. Allow the placenta time to separate without pulling on the cord.
4. If the woman is upright gravity will help, sitting on the toilet usually 
works well.
5. Wait until the woman feels some afterbirth pains, then suggest she gives 
a little push
6. Always observe for excessive bleeding but dont confuse that initial gush 
which indicates the placenta is separating with excesive blood loss.
7. Be patient, have faith in the normal process!

Hi,
I'm interested if there is any research on delaying synto for say up to 5 
minutes in 'active 3rd stage'.
Have been doing actively managed third stage throughout my clinical 
placements as a student (nearly finished!!) with some practitioners cutting

the cord immediately, and most at about 10 - 20 seconds.
I've just prepared a powerpoint presentation on delayed cord clamping but 
know I will get into a discussion around the seeming conflict between 
active 3rd stage and delaying the clamping. Obviously if you don't want the

effects of synto's action - strong uterine contraction with excess blood 
being pumped into bub, then you need to delay the entire process of 
actively managed 3rd stage until the cord is clamped.

Does anyone practice delaying the synto injection for those first few 
minutes? Any evidence of harm in doing this?

Just going from a very faded memory from
my Mid education, but wasnt it the Hinchinbrook trial that outlined the
dangers of mixing the two practices? I know it was seen as a flawed and dated
trial, with dubious parameters of normal and of what physiological actually
meant, but if someone can find the paper it might lead to other references

I have a few births at home coming up and was 
wondering about synto and other drugs in my kit. How do others purchase them? Do 
I have to have a script from a doctor? The other issue that I do find difficult 
is the issue of cost for homebirth.Others I have been involved in have been for 
friends and colleagues.Does anyone have a schedule of payment and cost 
that they use? I am meeting with a couple on Monday and would love to have a bit 
more idea. Any feedback will be greatly appreciated,

As a woman planning for a homebirth, I
just went to my GP and got a prescription from her for a dose of syntometrine.
I still have to go to the pharmacy to get it filled- hope they are as easy to
deal with as my GP is J I do know my midwife has a GP who will write scripts for her, yet my
GP is supportive of HB and I was happy to approach her for it.

could be a good start as they have some good scales and are much cheaper

This is part of the big problem with the
way in which independent midwives are forced to work here in Australia isnt
it? An IPM who chooses not to carry synto or erg would be crucified in the
event of a PPH, but the reality is that there are few ways to legally acquire
it, and even fewer ways to legally administer it. Sadly we have a long way to
go in this battle, and though it seems like a miniscule thing, I think its a
big deal that many midwives simply have no way of getting the drugs they need
to carry, and equally no way to be able to give them when the are needed. I
dont know of one single Dr in SA who would write a standing order for synto
knowing that the woman receiving it was choosing to birth at home. And why
should women have to broach the subject with (mostly) disrespectful and
ignorant GPs. Isis, you are lucky!

[ozmidwifery] Birth After C-Section Information and Support group.

Hello,



I am contacting you to let you know that I am forming a Birth
after C-Section information and support group. The idea behind this group is to
give women (and their partners if they would like to) an opportunity to speak
to others that are interested or experienced in VBAC (Vaginal birth after 1 or
more C-Sections) and the options available. We will explore the facts, the
myths, the fears and the emotions that surround this decision and it will be a
supportive place for people to search for answers. We welcome those that are
already pregnant and those who are considering their options for a planned
pregnancy or anywhere in-between. This group will run once per month unless the
group would like to meet fortnightly instead. In order to run in the best
interests of all we would prefer that participants give themselves a couple of
child free hours to focus on themselves, however we welcome babies.



The first gathering will be on Thursday the 23rd
of November at 7:30pm.

Location - Shop 6/16
  Brampton Street, Cranbrook
in the Kids in Harmony Centre.

of November (the meeting will only go ahead if you let me know youre

  to the greater maternity reform agenda too lose in unrelated hospital issues. 
  Whilst I am disappointed for the women who are birthing now and will be 
  affected I think sometimes we have to weigh up issues for the greater 
  good.At the same time we need to support the women in the here and now 
  (this is something I have always arued for when bureacrats talk softly softly 
  on reform)There are some options. If any woman wants to stand 
  their ground let us know and we can support her. Otherwise I think we 
  really need to insist that these women have the same facilities they could in 
  Ryde ie access to water etc etc. Obviously they will still have their 
  known midwife.Please let us know if we can assist in advocating for 
  these women as MC was part of the establishment of the Ryde service and we

Andrea, it is my understanding that one
still has to have a Drs order (e.g. prescription) before a midwife can
actually give the drug. Prior to the CMP being under the umbrella of the State
Health Dept, WA metro midwives had to get the woman to get a script from their
doctor for synto, Vit K and xylocine 1% for suturing. There are few doctors who
will actually do this. Strange, they say you might bleed to death
at home but wont give any help in preventing this scenario. Good luck,

Had a client recently who I sent to her GP for a script for synt. She 
got the script, went to the chemist to fill it  found it was going to 
cost her around $80 to get it - they only sold it in the boxes of five 
vials. I ended up asking around my MIPP friends  managed to find some 
that way (dint need it anyway so its still in my fridge).
If you give me your postal address Im happy to post some to you - my 
understanding is that its ok to keep it out of the fridge for a time.

Dear all
I might be able to shed a bit of light on Justine's comments. 
One of the obstacles we had in getting Ryde Midwifery Group Practice up
and running was an assurance to the Area health service that there
would always be someone (obstetric) capable of doing an emergency CS
if necessary at the Ryde hospital - in case of a dire emergency. 
Needless to say this has never been called upon - but with the
troubles of the Ryde General Hospital - the RMGP is drawn into it all
unwittingly - because the RMGP can no longer assure the area heath
service that an obstetrician can provide that back up at Ryde.
Therefore this is really a timely opportunity for the RMGP to reframe
the service they are providing and have it ratified by the Area health
service - without the nonsense clause that existed in the first
proposal. Belmont for example does not have this clause - and I think
Ryde midwifery group practice only had it because we had to compromise
a on a few fairly benign (we thought at the time) caveats in order to
get started. Now we are several years down the track and looking OK -
i dont think it will be such a hurdle..

so I think Justine is correct is asserting that it will be only a blip
in the process - but a process never the less - and for those of you
getting these things up and running - there are so many battles to be
fought -i guess we have to look at this as just another not
insurmountable (but tiresome) hurdle. 

I do think it would be incredibly helpful if any consumers concerned
could take it up with as many people as possible - ie their local
member of parliament, the CEO of the Area health Service, the head of
Obstetrics at Northern Sydney Health, the NSW Health department , the
Australian College of Midwives - anybody who needs to know this isn't
good enough  best wishes

any references for this?
Any idea what occurs if the placenta takes longer to arrive than the 20

Hello Sue,

The question of third stage management has a cultural aspect as well. 
In the US, as far as I know, the syntometrine is not given until after 
the placenta arrives.  It is then given to prevent excessive 
bleeding.  Interesting to speculate on how this major difference 
developed, and why  it is acceptable to wait the 20 or so minutes for 
the placenta to come physiologically in the US when it is unacceptable 
in the UK/Australia.


Another one of those examples of how habit/routine becomes standard 
practice and is not questioned.

Hi Cath,

I give the women a letter for the doctor. We've ( 
the woman and I) have already discussed which tests she would like/ wouldn't 
like to have. The doctor is then given the letter with an outline of what the 
woman needs. This can be basic bloods, a script for vitamin k, a script for 
synto, a letter for O2 hire, ultrasound forms etc.

On costing, it a hard one. I found when I first 
started out, it was hard to charge, and it was a 'self worth' issue. A few 
REALLY long births soon changes that!! In Sydney, most independent midwives 
charge between $3500 -$4500. This will also change for repeat clients, primips 
vs multips, distance to be travelled etc.

Yes, the synto is about $100 a box. So what I do, is buy/pay for one box, 
which lasts for the next women ( does that make sense?), I only use Synto 
about once a year! ( and then there are the years you need it 3 times in a 
row!)

Had a client recently who I sent to her GP for a script for synt. She got 
the script, went to the chemist to fill it  found it was going to cost 
her around $80 to get it - they only sold it in the boxes of five vials. I 
ended up asking around my MIPP friends  managed to find some that way 
(dint need it anyway so its still in my fridge).
If you give me your postal address Im happy to post some to you - my 
understanding is that its ok to keep it out of the fridge for a time.

I have a couple of questions about this situation in regards to the media 
statement ~

 Unfortunately it has not proven possible to recruit to Ryde Hospital a Career

It was my understanding that midwives have all the skills necessary to assist 
resus of a baby, am I mistaken?
 
 Therefore, effective immediately, all women booked to birth at Ryde  
 will birth instead at Royal North Shore Hospital, in the care of their 
 attending midwife.  

I just wanted to comment here that I find it infuriating that the assumption is 
made that 'all women booked to birth at Ryde will instead birth at RNSH'. What 
about what the women want, they weren't even given a choice!
 
 I regret that this short-term arrangement is necessary but labour and 
 delivery can be unpredictable even in low risk situations. It's important to 
 have medical expertise available if required.
The safety of mothers and babies is our priority.

Please correct me if I'm wrong, but don't all midwives have the 'medical 
expertise' required?? 

I know I'm preaching to the converted, but Obs must just wonder how on earth 
our human race has survived without their 'expertise'. Sorry, but I think this 
is bullshit! 

Surely it hasn't all of a sudden just happened. I mean, how long has this been 
known for? Shouldn't the women booked in have been informed that this could be 
a possibility and given the opportunity to change to another birth centre? This 
temporary situation is affecting womens lives and it's wrong. They have had 
their right to a choice denied!

Thanks Justine. I will pass on your details to my sister. I'm not sure if she 
will contact you though, she is completely shattered. Her last birth was 
horribly traumatic in a hospital as was the aftermath. She is freaked out but 
doesn't feel like she has the strength to fight it.

Hi Cath,

 I do a similar thing to others on the list, ask the OB I work with for a 
script for any drugs needed ie synt, ergo, konak, xylocaine, maxalon, anti-D, 
Hep B Vax etc. Client gets script filled  I buy them off her if I don't have a 
cache myself. I use these things so rarely that 1 script per year is enough. He 
will also fill out a medication chart if I ask him so I am giving a drug with 
an order if needed. All nicely legal.

I have all pre-signed path  ultrasound forms from the same OB  use them at my 
discretion. Rare. I'd usually phone him  get the OK out of courtesy before 
using them as he is so supportive I feel it is an important part of 
collaborative care.
 
Costs for HB down this way :  $1600  to  $2500 approx for the entire package.

I give the women a letter for the doctor. We've ( the woman and I) have already 
discussed which tests she would like/ wouldn't like to have. The doctor is then 
given the letter with an outline of what the woman needs. This can be basic 
bloods, a script for vitamin k, a script for synto, a letter for O2 hire, 
ultrasound forms etc.

On costing, it a hard one. I found when I first started out, it was hard to 
charge, and it was a 'self worth' issue. A few REALLY long births soon changes 
that!! In Sydney, most independent midwives charge between $3500 -$4500. This 
will also change for repeat clients, primips vs multips, distance to be 
travelled etc.

misoprostal isn't licenced here is Australia.  I wouldn't be prescribing it 
if I were a GP.  When I was Working at a private Hospital  the Obs kept it 
in their own possesion.  It isn't licenced to be kept at the hospital as far 
as I know.  The pharmacy at the hospital wouldn't touch it.  It's not the 
sort of drug you should have at a homebirth anyway.

Had a client recently who I sent to her GP for a script for synt. She got
the script, went to the chemist to fill it  found it was going to cost
her around $80 to get it - they only sold it in the boxes of five vials. 
I



ended up asking around my MIPP friends  managed to find some that way
(dint need it anyway so its still in my fridge).
If you give me your postal address Im happy to post some to you - my
understanding is that its ok to keep it out of the fridge for a time.

We use Misoprostol at the hospital where I work and it is kept in the fridge 
next to the syntocinon and syntometrine and the prostins etc.


Why would you want to use it at your homebirth but. Syntocinon should be 
just fine. Misoprostol is a fairly heavy drug of choice with a fair few side 
effects and we only use it for large PPH's
