I've sent him my 6-25-73 discs at least three weeks 
ago.
I've sent several messages and he is NOT 
responding.
ALSO, Gabriel Artur sent him some discs and is NOT 
getting any response either, and he has apparently had more than ample time to 
reply. Mark D. was supposed to receive these discs from this guy and never 
has.

So DO NOT send anything to this guy or include him 
in any vines, weeds, trees etc as he does not appear to be willing to comply 
with upholding his end of the deal.

-FZ wrote the liner notes. That's either a typo, or FZ misremembering (it happens, see YCDTOSA liner notes). It's from sessions that ended up comprising some of Apostrophe.
My other question is can anyone tell me of any other source that this song was ever released on - or ever performed live by FZ, Keneally or anyone?
-Not by FZ. Don't know if anyone covered it live or not.

Let us not forget that the reason you're sending me the Jazz from Hell
disk in the first place, is that the one I was sent (can't recall from
whom, at this very moment), was broken in transit to me. If that was
one of only a few copies, it could be a contibuting factor in it's
scarcity...

 that also included the GTOs Permanent Damage in wav format. That

 misplaced/lost the disc for a good week and a half and thus took a

 to him than I should have). So at least it's still out here
