'extra-prompt.sh' script.
If I include the 'extra-prompt.sh' script, I get two prompts, one after the
other. I must assume, therefore, that the 'extra-prompt.sh' script is included

Err, have you been following lfs-book at all? If so you'll have seen
there's been a reasonable amount of work done on the 6.1.1
