Ok, first things first, yes I have owner rights to the Org Forms Library,
second I started the whole thing
We have a form that resides in our Organizational Forms Library that we are
constantly updating, but for some reason everytime I make a change and then
subsequently re-publish the form, the next time I open up the form it is the
same as before I made any changes. It has happened several times

or not? Also, when you're publishing make sure you do
Forms\Publish Forms As and browse to Organizational Forms Library as opposed
to clicking on the tempting Publish Form button.

Have you tried deleting the form from Org Forms Library? Can you delete
it? Have you then tried republishing it using a known good copy of the
form?
