Ok, filed! I've included a much smaller document.

You're right: RTF is probably not the most important part of FOP for
everyone, but for some people it is. Everyone has a chance to help
improve the parts he/she is most interested in. That's the cool thing
about open source.
I'm a little confused about your post. Let's look at the details:

You can use any private namespace URI for private elements and
attributes. Just make sure you don't use the FO namespace. However, in
both cases FOP will complain on the log but continue without problem. In

Yes, it looks like the Ant task wasn't looked after properly. I've
reconnected the configuration in FOP Trunk:
http://svn.apache.org/viewvc?rev=408707&view=rev
It could probably do with some extra attention. For example, doing XSLT
and FOP in one step to improve performance and to avoid temporary

No, don't replace fo:table-and-caption with fo:table, remove all
fo:table-and-caption elements.

Confirmed. Seems to be a bug. Unfortunately, I won't have time
immediately to have a closer look.
If your document could be cut down to the minimum so the disappearing
footnote can still be reproduced, that'd be great.

Hi:
Does FOP have any support for user-defined tag or attribute
for fo:table so that this extra tag is not to be interpreted by FOP engine but
can be used by the user for keeping some information for future use. The
information needs to be embedded within xsl-fo document.
Regards,
Sorry I didn't understand that. Is it possible you can provide a link with an
example or explanation?

You can achive that with two transformations. The first to transform
the above in:
and the later as usual, to produce the FO to pass to FOP.
