In the PDF Setup dialog box, go to the Links tab and check Create Named
Destinations for All Elements and Paragraphs and see if that makes a difference.

They're all links to external URLs. This option just adds data to the
potential destinations within the given PDF file.

Well, it turns out that it's the PDF Preview that ships on the Mac that is not
recognizing hte links. Still it's odd -- what is different that the same links
work or don't work depending on where they are in the document when I generate?

One additional note: "Why continue to use the Save-As method? Why not just keep the process I’ve always had? Well, for one thing, when I have to produce the entire document (over 30 books), it is much faster and simpler to use the Save-As PDF."
