The other possibility is that
tools-extensions shows just the users extensions.
while the stand alone unopkg gui shows all of them.

I am able to reproduce this in LibreOffice 3.4.3 OOO340m1 (Build:302) in KDE
(Kubuntu 11.10). Uninstalling libreoffice-kde seems to fix the problem.
