The problem is the obsolete LaTeX-package a4 in the LyX document preamble.
Remove it and it will work.

With all the recent buzz about Lyx 2.0 I am curious to try it. I don't
like running two versions in parallel, and 1.6.5 works, but if Lyx 2 had
solved the very annoying problem with Change Tracking making it so messy
to move figures around a manuscript, I would be eager to install Lyx 2.
