If you want to put the notes at the end of each chapter, you don't need the endheads package. Endheads is for adjusting the running headers in endnotes sections at the end of the book, so that it will say something like Notes to pp. 142 -- 7. Endheads can organize those notes by chapter, and

I just confirmed that on Mint (Linux), LyX 1.6.4 ignores the personal dictionary setting when using aspell. (I did not try any other spelling checkers.) There also appear to be a couple of other bugs involving the associated dialog. I've reported all the bugs. Meanwhile, as Stephen said,

I'd suggest you delete your fledgling layout file (you were on the right track, but you need more detail than just the header), then have a look at the LyX layout file provided at http://www.scss.tcd.ie/Eamonn.Linehan/code-latex.php. Modify Dr. Linehan's layout file to point to your thesis .cls
