You need to hear VoiceOver say “Placeholder selected” to know that the
placeholder text is actually selected as it reads the placeholder text even
when it isn’t selected. You can also use VO-F6 to check that it is selected. It
needs to be selected to type over it.

To replace placeholder text, you need to navigate with only the arrow keys so
that the whole placeholder text is highlighted before you start typing over it.
The whole piece of placeholder text will then be replaced.

When you want to modify placeholder text to create new placeholder text, you
have to set it back to normal text then edit it as usual and then turn it back
into placeholder text.
