press vo/shift+m and then arrow up once to download episode. Press return, and

Actually I tried that and hitting enter on the episode I wanted never started
the podcast downloading at all. I looked in the queue and still nothing. I do
have to arrow up to download episode. Strangeness.

Make sure the view is set to text list view in iTunes. It's a radio button
you'll find when you use control-option-left or right-arrow to move around the
screen. Just check it with control-space.
